Fixes found in a live test with Claude Haiku 5.5. No config change (config-version stays 8).

• Fix: learning from the chat often missed the second mention of a fact, so nothing ever reached the review: the model reported confirmations of known facts under keys of its own (fact_hits, server_confirmations...). The rules now show the exact format, and id references under another key are accepted too
• Fix: a player who mentioned a fact in the same round as another player was missing from the fact's player list in the review
• Fix: the model no longer writes comments ("nothing new here") as facts
• Fix: warning at every start that the bot's self-description entry is longer than 600 characters and was cut (the list of features on/off is now two entries)
